How To Get Started

Thank you for visiting. All of my initial consultations begin with a comprehensive blood chemistry screen, followed up with a 30 minute consultation to review your blood work. This consultation also includes a health history intake, initial recommendations based on your blood work, as well as a future plan. A sample of the Functional Blood Chemistry Analysis packet you will receive during your initial visit can be found at the bottom of the page.

The cost of this Wellness Starter Package is $189, which includes all laboratory fees. To get started and to make an appointment, please follow the instructions below.

Meadowhill Wellness Center (Annapolis)

1. Call the front desk and schedule an appointment (410.263.0411).
2. Once you have made the appointment, download and fill out the
   Basic Intake Form here. (You must have Adobe Reader, available free from Adobe.)
3. Fax in the entire form to 734-468-8573.
4. Within one business day you will receive an email with the subject line "Confidential: Your Lab Requisition" that will contain the form to bring to the nearest LabCorp drawing station. Print out the form and bring it with you when you go to get your blood drawn.
5. The results of your intake form and blood work will be reviewed during your appointment.

VIS Vitality and Wellness (Columbia)

1. Download and fill out the Basic Intake Form here. (You must have Adobe Reader, available free from Adobe.)
2. Fax in the entire form to 734-468-8573.
3. Within one business day you will receive an email with the subject line "Confidential: Your Lab Requisition" that will contain the form to bring to the nearest LabCorp drawing station. Print out the form and bring it with you when you go to get your blood drawn.
4. Click here to book your appointment online. Important: Select a date no earlier than one week after you have had your blood drawn to ensure that your results are back before your appointment.
   1. Click "Register", fill out the information and hit "Submit".
   2. Log on to the site.
   3. Select "Dr. Julie Kniess"
   4. Select "Initial Visit"
   5. Choose the date and time that works best for you.
5. You will receive an email confirmation once you select an appointment. You can cancel or change the appointment 24 hours prior to your scheduled time.
